Most local distribution companies require an authorization or consent form prior to change of service. Click on your local gas utility below in order to print a form or to find out what is required for enrollment. If your local gas utility is not linked to a form, contact your regional sales representative as you may not need an enrollment submission. Please mail the completed form directly to UGI Energy Services so we may process the paperwork with your local gas utility.
